Catchment Area
New Longton Surgery extends a warm welcome to new patients who live within the practice area.
Before filling out a registration form, check you are in our catchment area by using the map and postcode checker opposite.
Registering with the surgery
- Please use the online service called Register with a GP Surgery that makes it easy to register with us. This service will also check your address is within our boundary. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please ensure we have your mobile telephone numbers and email address as we now send out appointment confirmations, reminders and blood results using these methods. If you do not wish to receive SMS messages you must OPT OUT please let reception know.
Before switching Practices please ensure you have enough medication to last for at least 2 weeks.

Download the NHS App from the App Store (iOS) or Google Play (Android).

Open the app and tap Continue with NHS login.

Enter your email address and tap Continue.

If you already have an NHS login, sign in with your existing credentials. Otherwise, proceed to create a new account.

Create a password and tap Continue. Check your email for a verification link and click it to confirm your email address.

Enter your UK mobile phone number and tap Continue. A 6-digit security code will be sent to your phone. Enter the code to proceed.

Prove your identity by either: Uploading a photo of an accepted photo ID (e.g., passport, UK driving licence, or biometric residence permit). Using registration details from your GP surgery (e.g., Linkage Key, ODS Code, and Account ID) if you don’t have a photo ID.

Follow the on-screen instructions to complete the identity verification process.

Once your identity is verified, log in to the app using your email and password to access NHS services.
Temporary Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access:
- collect a proxy access form from reception
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Address/Telephone Number
If you move house or change your phone number please let us know. We hope that you do not move too far away, as we would like to continue to provide care for you. It is useful for us to have your mobile telephone number as we can use this to send you appointment reminders, and test results.
